Admission Procedure

As an in-patient of the Indian Spinal Injuries Centre, you have the right to receive considerate, respectful and compassionate care in a safe setting. You also have the right to ask your attending doctor to explain your diagnosis and the line of treatment. For seeking admission to the hospital you would need to follow the procedure outlined below.

Admission Request Form

The request for admission is required to be made on an “Admission Request Form” available with all the consultants and at the Front Office. The consultant recommending admission will make a ‘provisional estimate’ on the performa printed on the forum and authenticate the same with his signatures.

Admission Sheet

Having received the provisional estimate from the attending consultant, next step would be for the patient/ authorized attendant to get the ‘Admission Sheet’ filled up at the ‘Admission Desk’ in the Main Lobby and sign the authorization for medical and surgical treatment

Allotment of IPD No.

Once an admission sheet is approved an In-Patient Department Number will be allotted and the patient/ authorized person will be required to fill up and sign the consent form authorizing medical treatment, administration of anaesthesia and performance of procedures/operations.

Classification as TPA Entitled Case

As in-patient staking claim to enjoy third party assurance cover would be verified by our functionaries manning the “Corporate Desk” (LINK) and would endorse billing instructions on the admission documents accordingly.

Billing Sheet

Date, services, rendered and the charges for the same will be recorded in the billing sheet for all fees for consultation, operation charges, investigations, treatment and food and beverages.
